** The CMHA does not register players who are under the age of 5 as of December 31st, 2010.
** PLAYERS ARE NOT CONSIDERED REGISTERED UNTIL THEY ARE PAID IN FULL.
** MEMBERS WITH OUTSTANDING DUES FROM LAST SEASON WILL NOT BE CONSIDERED REGISTERED
TO THE 2010-2011 SEASON UNTIL THE BALANCE FROM LAST SEASON IS SETTLED.
IMPORTANT NOTE: All returning players must be registered with the same first and
last name that they were registered with last season, including hyphens and/or spaces.
ie. John Doe cannot be Johnny Doe, MacDonald cannot be Mac Donald.

CLARIFICATION ON MIDGET JUVENILE LEVEL: For the purpose of registration,
all players in birth years 94/93/92 register as Midget Juvenile.
Divisions are as follows:
HOUSE LEAGUE: MINOR MIDGET 1995 & MIDGET JUVENILE 1994/1993/1992
(divisions will be grouped based on final registration numbers)
REP: MIDGET 1994/1993 (we do not field Juvenile Rep teams)
SELECT (Intra-City): MIDGET 1994/1993 (we do not field a Juvenile Select team)
PAYMENT OPTIONS:

REFUND POLICIES:
A cancellation/refund request must be emailed to the hockey office at tcmha@on.aibn.com.
House League & Select refund policy:
- Non
refundable administration fees - $15 for Hockey School and $25 for all
other divisions.
- Full
refund less administration fee up to Oct 31
- 75%
refund less administration between Nov 1 and Nov 30
- No
refund will be issued after Nov 30
Rep refund policy:
- $50
non refundable administration fee
- 100%,
less $50 administration fee (less $50 late fee if applicable) up to Sept 15
- 75%,
less $50 administration fee (less $50 late fee if applicable) up to
Sept 30
- 50%,
less $50 administration fee (less $50 late fee if applicable) up to October
15
- No refund will be issued after October 15
Child Fitness Tax Credit
**Please
keep a copy of your automatic receipt in a safe place as it will be the only
receipt issued.
